*Use this form if: You have completed additional college hours or a degree program. This form and transcript must be received by Human Resources no later than Sept. 1.
Notification of Additional College Hours/Degree Late Transcript Form**
**Use this form if: You have completed additional college hours or a degree program but are unable to obtain your Official Transcripts reflecting such by September 1 but will have them no later than December 31. This form is due to Human Resources no later than September 1.
Name Change
Before last names can be changed, you must change your name with Social Security and present that documentation to Human Resources.
